VBS Registration Form Template

At our Vacation Bible School, parents fill out one registration form for all the kids in the family.  This form is used for our VBS program, then slightly adapted for all other children’s ministry activities throughout the year. Download our VBS registration form below to reference for your church events.

FYI – We also offer the choice to pre-register. Our pre-registration is done on Google Forms and collects the same information as the paper registration form below.

IMPORTANT: There is a line on the registration form for allergies.  Make sure your registration team pays attention to this information and alerts the appropriate volunteers (crew guide, snack team, etc).

To customize the form for your church, download the Word document or PDF to your computer then change any information to meet your needs.  We typically use colored paper for the registration forms so they’re easy to spot in the piles of paperwork that can tend to accumulate around the church office {especially at VBS time!}.

We also have a different registration form/waiver for events where parents attend the entire event with their children.
