I am by no means a lawyer or anything close to it, so please consult one before depending on this registration form to save you from a lawsuit! 

In previous years, our church had registration cards {printed half sheet on cardstock paper} that we used every year for VBS.  However, there wasn’t much of a waiver on it {if any at all}.  As times have changed, we decided it would only be responsible to add a waiver to the registration form.

At our Vacation Bible School, parents fill out one registration form for all the kids in the family.  We offer the choice to pre-register, but most families come early the first night and register on the spot.  {This year we had 85 kids at VBS.  I believe 6 or 7 of those were pre-registered.  It would make planning much easier if we could get more people to pre-register, but we haven’t pushed that yet.}  There is space for four children in the family on one registration form.  If a family has more than four children, we have them fill out the “child” section of a second form, then staple both together.

IMPORTANT: There is a line on the registration form for allergies.  If you register children on the spot, make sure you are looking through the forms to check for allergies and alerting crew guides, snack team, etc if any exist.

This form is used for our VBS program, then slightly adapted for all other children’s ministry activities throughout the year.  I’ve attached a PDF of the VBS Registration Form Template {I just removed all of our church information and replaced with blank lines to type or write your church name in}.  Feel free to change anything about it to make it better fit your program!  If you’d like to customize the form, just download it then change any information to meet your needs.  We always use colored paper for the registration forms so they’re easy to spot in the piles of paperwork that can tend to accumulate around the church office {especially at VBS time!}.
