Family Night Registration Form Template

This year we had a VBS Family Night for the first time! It was a FANTASTIC event that we are planning to continue in the future. One thing we had to create for family night was a registration form/waiver. Download it below to reference for your church events.

IMPORTANT: This event registration form is intended to be used only when parents are present with their kids throughout the entire activity. For activities when parents are not present, we use a more thorough registration form.
